Creating a Culture of Quality and Safety to Reduce Nosocomial Infections
Outline:Abstract:
The persistent high rates of nosocomial infections indicate that traditional approaches to prevention have failed, and that these infections are not primarily a knowledge or technical problem, but rather a complex one requiring social and behavioural change. Traditional best practices and their enforcement either fail outright or achieve only modest and often non-sustained and unscalable improvement. Industrial tools for quality management and improvement, such as Lean and Six Sigma, are only successful if accompanied by a cultural change. Creating an optimal culture of quality and safety requires many different techniques from top-down, bottom-up, and sideways-in. We describe Positive Deviance, a behavioural and social change strategy, and its adoption at the VA Pittsburgh Healthcare System to reduce nosocomial infections. Positive Deviance enabled those whose behaviours needed to change, to self-discover the behaviours and practices that could eliminate transmission of infection in their area of work.
